Medical insurance plans and payment options

We are currently contracted with Blue Shield, Blue Cross, Cigna, United Healthcare, PHCS (Private Healthcare Systems), CCN, Admar, and First Health. Unfortunately we do not accept HMO plans. If you have a POS plan, you will need to provide authorization to your carrier to pay the claims under your out of network benefits. We do not accept Workers' Compensation patients.
